What To Expect On Your New Patient Visit

To make the transition to our practice efficient and convenient, we have put together a few directions for actions you should take prior to your first visit; however, please be assured that we are available during our normal business hours to address your personal needs. Thank you for choosing us for your care. 

Request your previous medical records, if applicable.

​

  • If your first appointment with us pertains to an established problem for which you have received medical care elsewhere, please arrange for transfer of these records so we have them in hand on the day of your appointment. This will allow your practitioner to provide continuity of care and will reduce unnecessary duplicity in diagnostic measures or treatments. 

  • If your first appointment is for a routine exam or a new problem, we do not require your previous medical records. However, you are welcomed to transfer your records if they contain pertinent medical history.

Verify your insurance benefits

​

  • It is best to call your insurance company to verify your benefits prior to being seen. 

  • If you are unsure if we accept your specific insurance plan please reach out to us either by our contact form or by phone to confirm.

Things to bring with you to your first visit

​

  • Insurance card(s) (Please plan on bringing your card(s) to every visit). 

  • Your copayment. We accept cash, check and credit card. 

  • Picture ID (driver’s license, school ID, etc.).

  • Medical records, if applicable (see above).

HIPAA

Your Care

All adults should have a complete physical exam with blood pressure monitoring yearly. If you should fall off schedule, we will contact you to schedule an appointment.


Annual wellness visit/physical exam appointments require more time in the office therefore are more
difficult to fit into our providers schedules. Typically, we will be scheduling 2-3 months out from the

time of your call so please plan ahead. We encourage you to re-book your yearly physical exam for the
following year before leaving the office on the day of your visit.


Non-urgent follow up visits may take a week or more to schedule as we need to maintain availability to see patients with urgent problems.

Urgent Care

​

Patients with urgent issues such as minor injuries, sore throats or similar conditions are typically seen
within a day or two. We will offer you the soonest available appointment, however we ask that you be
flexible as you may not be able to see your assigned provider. Please call the office as early as possible in
order for us to see you for the first available appointment. Certain medical problems such as acute chest
pain, broken bones and serious injuries cannot be treated in an office setting and may require you to
visit the emergency room. If we are unable to accommodate you with an appointment we may refer you
to an urgent care facility. 

 

Our practitioners are available after hours please limit these calls to genuinely urgent needs. If you fell and this is an emergency please dial 911 immediately.

Refill policy

​

Please contact your pharmacy directly for refills of your medications. Plan ahead for your refills it
may take 24-48 hours to process a refill request. Please note that prescriptions for controlled substances
may take longer to process as they may not be called into the pharmacy.

Referrals

​

On occasion your provider may refer you to a specialist. If your insurance requires a referral from our
office, please have the following available: Name of physician and/or practice, first date of your visit, NPI #
of physician you will be seeing, and diagnosis for visit.

 

Please leave all information on our referral line at extension 491. Please keep in mind that most insurance companies will only allow us to back date a referral for 90 days, so please contact us with the necessary  information as soon as you are aware a referral is needed.


Our specialty scheduling department works closely with area specialist and we will work to get your
appointment as soon as possible, please allow two weeks for appointments to be scheduled. If you have
not received an appointment after the two-week time frame, please reach out to our scheduling staff at
ext. 102
